Personal stories illustrate the progress of the Putting You First project in its latest update report
Thursday, 12 September 2013 11:53
Chair of the Community Health and Social Care Partnership Board, Councillor Andy Ferguson, said, “There are currently 72 Putting You First projects underway, ranging from very small trials, to larger changes. This is an impressive range of work.
“The key is early intervention – changing the way we work so that people can be independent and in their own home for longer. Dumfries and Galloway is showing some success in achieving this. Change is not an option. It is vital if we are to transform services and make them sustainable as the population ages.
“The stories of the people involved in the projects are moving and show how simple changes can improve people’s lives dramatically. Many of these ideas will be developed further as we begin the wider integration process.”
Andrew Johnston, Vice Chair of the Community Health and Social Care Partnership Board and also Vice Chair of the NHS Board stated “The stories show how the Putting You First Programme is working in partnership to test changes that will assist as services change in the future. To ensure that the tests are having a positive impact on people, an extensive evaluation is currently being undertaken.”
Putting You First is a five year change programme which manages the Dumfries and Galloway share of the national Change Fund. It is at its mid point. The Change Fund is there to support the Scottish Government’s ‘Reshaping Care for Older People’ Strategy.
This local partnership brings together Dumfries and Galloway Council, NHS Dumfries and Galloway and the Third and Independent sectors and seeks to:
• Develop flexible services that respond to people’s lives and their changing needs
• Let people be partners in their own care and support them to make their own decisions
• Provide services as close to people’s homes as possible
• Ensure staff and partners have the right skills and resources to make this happen
Members of the Community Health and Social Care Partnership Board noted the structural changes in the Putting You First work streams, the positive feedback from individuals and staff who have taken part in the tests of change and the wide range of work that is being carried out.
Case Studies:
The Dumfries Health & Social Care Hub
This project has brought staff together from Dumfries and Galloway Council, NHS Dumfries and Galloway and the Third Sector. It is testing new ways of providing early intervention and intermediate care for the over 65 opoulation of the Greyfriars and Charlotte Street medical practices. the Hub has already had some successes in allowing people to stay in their own homes, by the STARS team, social work and health all working together.
The Hard of Hearing Group
This Annan group is funded by Putting You First and is staffed by NHS trained volunteers. they offer a bi-monthly care and repair service for hearing aids. It reduces the pressure on Audiology at DGRI and has speeded up the time people have to wait to get their hearing aids replaced. For example, this allowed one particular person to settle into his respite care easily. He could hear what was going on around him and knew what was going on.
The IDEAS team
IDEAS stands for Interventions for Dementia, Education, Assessment and Support. NHS Mental Health Directorate has received funding for this team to improve the quality of life and management of people with dementia. Care home staff who have received training have commented:
'The training gave me a better insight into dementia and the people I will be helping'
'I have to say it opened my eyes on how to approach residents.
